The mission of Addison County Home Health & Hospice, Inc., a non-profit agency, is to provide high quality, comprehensive community health care to residents of Addison County.

THE MISSION OF THE DAUGHTERS OF SARAH NURSING CENTER, INC. IS TO ENHANCE THE QUALITY OF LIFE, ASSURE THE SAFETY AND DIGNITY, AND FOSTER THE INDEPENDENCE OF THOSE WE SERVE THROUGH RESIDENTIAL, PERSONAL CARE, HEALTH AND OTHER QUALITY SERVICES, CONSISTENT WITH JEWISH VALUES AND TRADITIONS BY OPERATING A SKILLED NURSING HOME AND A SUB-ACUTE REHABILITATION CENTER FOR ELDERLY INFIRM AND DISABLED ADULTS IN NEED OF ASSISTANCE WITH THE ACTIVITIES OF DAILY LIVING. THE FACILITY IS LICENSED AS SUCH BY THE NEW YORK STATE DEPARTMENT OF HEALTH IN ACCORDANCE WITH THE NEW YORK STATE PUBLIC HEALTH LAW.

The mission of New Horizons is to provide food and shelter to people in need and offer supportive services to achieve self-sufficiency.

Spiritual love and concern for the terminally ill are reflected in our work at Haynes House of Hope. We open our home, our hearts and our hands to those who require physical care, emotional support and spiritual comfort, regardless of race, color and creed without charge. We feel blessed to share in the journey and privileged to be present with the family and loves ones.

Patient AirLift Services (PALS) is a 501(c)(3) nonprofit charitable aviation organization. Our mission is to connect the charitable aviation community to people in need. We arrange free air transportation for individuals requiring medical diagnosis, treatment or follow-up, for compassionate assistance, for military personnel/family requests through the PALS for Patriots Program, or for humanitarian purposes through the PALS Sky Hope Disaster Relief Program.

Our mission and raison d’etre is to facilitate access to and to create the first welcoming, safe and affordable housing for low-income LGBTQ seniors in Boston, including through a formal role in the development of such housing, as well as creating and defining onsite services and programming that addresses the needs of LGBTQ seniors and supports community space to serve seniors in the Greater Boston community.

The Foundation is passionate about helping seniors and their families find and use resources to enable aging with grace, independence and control. We promote informed decision-making to seniors and their families on a range of topics from care-giving to end-of-life care through webinars and in-person seminars/luncheons/meetings. We provide access to curated networks of retired physicians (called Physician-friends-of-the-family) as well as clinical pharmacists and counselors who provide compassionate counseling to patients and their families. We also provide financial assistance on to pay for short-term care-giving support.We believe that aging well is a group event, and in the company of family and friends, we can all age with grace and purpose!

LETS SHARE A MEAL INC IS A CHARITABLE ORGANIZATION SERVING FOOD (ALSO KNOWN AS "LANGAR") IN VARIOUS HOMELESS SHELTERS ACCROSS THE TRI STATE AREA. THE IDEA IS TO SPREAD OUR SAINTS MESSAGE OF "VAND CHAKKHNA"(SHARE WITH OTHERS)

TO PROVIDE A SAFE COMMUNITY WERE GUESTS CAN FIND FOOD, COMFORT AND HOPE FOR THE FUTURE IN AN ATMOSPHERE OF DIGINTY AND RESPECT.

BRISTOL HOME CARE AND HOSPICE IS COMMITTED TO PROVIDING THE HIGHEST QUALITY OF CARE TO PATIENTS USING THE CURRENT STANDARDS OF PRACTICE, WITH A FOCUS ON EDUCATION, PREVENTION AND SUPPORTING A SAFE HOME ENVIRONMENT. THE CLINICAL STAFF WILL PROVIDE SERVICES IN ACCORDANCE WITH STATE AND FEDERAL REGULATIONS.
