Their mission is to deliver nutritious meals to the aged, convalescents, homebound and handicapped persons who are unable to shop or cook for themselves, regardless of race, color, creed or ability to pay. They service the Main Line area of suburban Philadelphia, PA, from Bala Cynwyd to Malvern.

MEALS ON WHEELS OF SUMMERVILLE INC IS A COMMUNITY SPONSORED PROGRAM. ITS PURPOSE IS TO PROVIDE HOME DELIVERED NUTRITIOUS NOON MEALS SERVICE FOR THE ELDERLY, OR OTHERS WHO ARE HOMEBOUND, ISOLATED, OR INCAPACITATED TO THE EXTENT THAT GOOD FOOD PREPARATION IS NOT POSSIBLE.

Homesteads for Hope is a unique Community Farm experience for transitioning young adults with and without disabilities providing opportunities to learn, work, live and grow in the Greater Rochester Region. Our membership, programs, services and inclusive events are hosted on a 55 acre farm located on the banks of the historic Erie Canal creating a venue of dignity, purpose and sustainable agri-tourism. At LifePath Hospice, we go way beyond those basic hospice requirements to identify and meet the specific needs of each patient and family—from extra homelike touches in our two hospice houses to the hands-on bereavement care we provide to families long after their loved one has died. The mission of LifePath Hospice is to provide quality palliative care and relieve the suffering of those in our communities affected by life-limiting illnesses and end-of-life issues, maintaining the highest ethical standards, so all may live as fully and comfortably as possible.

Meals on Wheels Durham delivers daily nutritious meals plus friendly wellness checks to homebound older adults and other community members in Durham County so they can remain healthy, independent, and connected. The organization also provides related supports (holiday gifts, pet food, discharge meal programs, etc.) to improve clients’ overall wellbeing.

JCHE provides safe and affordable independent housing where older adults of all backgrounds can age in community. Our programs and services enable residents to live a full life of connection and purpose in a dynamic and supportive environment.

Palliative & Supportive Care of Nantucket Foundation is dedicated to increasing awareness and educating the community about palliative and supportive care issues as well as to funding all of the Palliative & Supportive Care Program's services and initiatives, all of which are provided to the community free-of-charge.
